To play on a team in the Minnesota High School Ultimate League, all players must provide the following information to their Coaches or Adult Team Advisors.  Go to this page (Player's Checklist) on the mnhsultimate.org web site to use the links.  Then fill out all the boxes, print and give the finished Checklist to your Coach/Advisor.

• USA Ultimate ID#: You must be a paid-up USA Ultimate member for 2012. If you are not paid up or not a member, click here for instructions to sign up and pay on-line or to get a mail-in form.  The USA Ultimate Membership Fee for High School students is $30. Pay this to the USA Ultimate, not your Coach/Advisor.
  
  
• Name: Your name must be the same as provided to the USA Ultimate or your Coach/Advisor won't be able to add you to your team's roster.
  
  
• Email Address: Optional but very useful.
  
• Phone Number:Also optional but also very useful. Cell phone numbers are preferred.
  
  
• USA Ultimate Waiver: Click here to log in to the USA Ultimate site to provide a Waiver.

• Medical Authorization Form: Click here for a Medical Authorization Form. Fill it in, sign it and have your parent or guardian sign it if you are a minor. Give this to your Coach/Advisor. Don't mail it to the USA Ultimate. Your Coach/Advisor is required to bring it to all games.

• League Fee: League Fees for 2012 are $40 per player. Give this money to your Coach/Advisor. He/she will write one check to the League to cover all players.

• Team Fee: Some teams have an additional fee that covers uniforms and other team expenses. Check with your Coach/Advisor.
